Care Coalition has been recognized as the Best Mental Health Systems Reform Model for Crisis Response and Family Support, marking a significant acknowledgment of its work addressing one of the most persistent challenges in modern mental health care: systemic fragmentation.
Most mental health systems were never built to work together, and the people who rely on them are the ones who pay the price. Care Coalition exists to change that reality through structural reform rather than incremental adjustment. Powered by Guardian Ad Litem Services and built on the proven framework of the Guardian Model™, the initiative unifies legal, clinical, and caregiving sectors into one coordinated, outcomes-driven ecosystem.
Developed and led by Michael Mackniak, a veteran attorney with over 30 years of experience at the intersection of probate law and mental health systems, the Guardian Model™ is a peer-reviewed care coordination framework that has demonstrated measurable, real-world impact. Its implementation has resulted in a 51 percent reduction in reliance on high-cost crisis systems, including emergency rooms, inpatient hospitalizations, and incarceration. The model has also been associated with significant cost savings at the state and community level, while improving long-term stability for individuals living with serious mental illness.
What distinguishes this mental health systems reform initiative is its dual-sided design. Care Coalition is one of the only platforms intentionally designed to serve both the family unit and the professional ecosystem in parallel. Families navigating mental health crises are often left to manage disconnected providers, unclear legal pathways, and inconsistent care plans, while professionals within the system are constrained by lack of coordination and structural inefficiencies. Care Coalition addresses both sides simultaneously, creating a shared framework where all parties operate from the same coordinated strategy.

The organization’s expansion into a broader national and global impact model is strengthened through its strategic partnership with Victoria Cuore, an 8-time international award-winning advocate recently named the #2 Top Women Leader in Empowerment for 2026. As the founder of A Contagious Smile and host of a top globally ranked podcast, she brings large-scale accessibility, emotional intelligence integration, and lived-experience-informed system design to the initiative. Through her work, she has built a global education platform offering more than 100 courses, many of which are free or low-cost, ensuring that financial barriers do not prevent access to critical knowledge.
Care Coalition maintains a strong public-facing presence through media and education, including the podcast Holding It Together (Kinda), hosted by Michael Mackniak, and the joint podcast This Is What It Takes, co-hosted by Mackniak and Cuore. These platforms provide direct insight into navigating complex mental health systems from both legal and caregiving perspectives.
The mission is clear and uncompromising: to replace fragmented, reactive systems with coordinated care structures that bridge legal, clinical, and family support. Care Coalition is not simply advocating for change—it is actively building the model that makes change possible, with a long-term vision of establishing care coordination as a standard expectation rather than an optional component of mental health services.
